All student-athletes are expected to have this grade sheet turned in to their coaches on the Thursday dates provided on the form.

Every student-athlete MUST have this form completed and on file with the ACTIVITIES office. For the 2011-2012 school year, the physical must be done AFTER May 1, 2011.

Each season students must complete this Emergency Care form and submit it to the Activities office prior to tryouts (along with the Extracurricular Participation form and the VHSL physical form-- the physical form only needs to be submitted once a SCHOOL YEAR).

All student-athletes must have this form signed (by both the student and a parent/guardian) and turned in to the coach before trying out for a sport.

Please read this form to help explain the details of the implementation of the Athletic Fee. Fairfax County Public School system has enforced this fee for all schools-- not just Falls Church HS.
